In order for us to best help your child in a time of need, it is important that we have all the required health records and personal health knowledge. Please make sure we have a copy of your child's immunizations, including updates on new vaccinations your child may have received, a completed blood lead form, a physical, the name and phone number of your child's physician, and any allergies your child may have or medications your child may be on.
It is vital that we have current phone numbers for contacts. If phone numbers change, please let the secretaries in the office know so we can update your information.

Vaccinations

No student will be admitted to school unless all the requirements of the immunization law are met.

Required Vaccinations for Pre-K Students

  • 4 doses of Diptheria, Tetanus, & Pertussis Vaccine
  • 3 doses of Polio Vaccine
  • Hib Vaccine (at least 1 dose after 12 months of age)
  • 1 dose of Measles, Mumps, & Rubella (MMR) Vaccine
  • 1 dose of Varicella (Chickenpox) or physician documented proof of disease
  • 3 doses of Hepatitis B Vaccine
  • 1 dose of Pneumococcal Conjugate


Required Vaccinations for Grades K-5
