The School Nurse reviews immunization records, which must be in accordance with Maryland Department of Health and Mental Hygiene.

In addition to:

• 4 DPT (diphtheria)
• 3 Polio
• 2 MMR (measles, mumps, rubella)

Students entering PreKindergarten, Kindergarten, and First Grade must have proof of:

• varicella (chickenpox)
• the Hepatitis B series

Students with less than complete immunization must have an appointment date for needed immunizations no later than 20 calendar days from the date of entrance.

Medication Administration

Specific guidelines are in place for those students who must have medication during school hours. Your school nurse will review the policies with you. These guidelines may also be found in the school handbook. There you will also find a Parental and Physician Authorization Form (P.S. 109)
