Attention : Parents of Students Entering 7th Grade in the Fall of 2016!!!


The time to act is now- make an appointment with
 

your child's pediatrician!

 

Here's what you need to know about the new vaccination requirements:

 

  • Each child will need -1 Dose of Tdap & 1 Dose of Meningococcal
  • Upon receiving these vaccinations, please provide your school nurse with the appropriate documentation prior to the beginning of the school year.

-If your student will be required to take medication at school, you can get the Physician Authorization Form from the school nurse or download the form from the St. Mary’s County Public School website. If the medication is a prescription, the form will need to be completed by the doctor. If it is an over-the-counter medication, we are only allowed to keep 2 doses at school with written permission from the caregiver. So, if you would like to bring in a couple doses of Tylenol or ibuprofen for your student, all we need is a note from you. However, if you would like to keep a whole bottle at school, then you will need to have the doctor fill out the form. The medication should be in the original container, not a baggie!
