REGISTRATION INFORMATION

Registration hours – 9:00 a.m. – 2:00 p.m.  Monday - Thursday

Please contact the school office at 301-863-4054 Ext. 0, if you have
any questions.

Please bring the following information with you to register:

● Birth Certificate
● Immunization Record
● Social Security Number
● Proof of Residency*
● Court orders, notarized statements, or other legally binding
instruments concerning custody, when custody of a minor student is an
issue.

*Documents accepted as proof of residence are: a lease agreement,
deed, current utility bill with parent/guardian name and 911 address
of the residence or current property tax bill. Monthly mortgage
statements, cable bills, and wireless phone bills will not be
accepted. If the child resides in a home that is not owned/rented by
the parent/guardian a notarized letter stating that the child resides
at the address is required from the person responsible for the
property. In this case, the document submitted as proof of residency
must match the name of the person signing the notarized letter.

Kindergarten Registration:
Kindergarten students registering for the 2012-2013 school year must
reside in the Park Hall school district and must be 5 years old by
September 1, 2012. The student does not have to be present at the time
of registration.

Pre-K Registration:
Pre-Kindergarten students registering for the 2012-2013 school year
must reside in the Park Hall school district and must be 4 years old
by September 1, 2012.
